The technology surveys below must be completed for every household, regardless of need, for state data reporting purposes. Chromebooks and hotspots are available to every student. To request a device, please complete the survey, indicating if you need a Chromebook, hotspot (internet access), or both. If you are requesting devices, please contact the office to arrange to pick them up after you have completed the forms.
