    All new and returning students to Jefferson Union High School District in 2017-2018 school year must submit an online registration form. Enrolling parent/guardian must have an active email address to register.

    Parent/Guardian will be asked to upload the following documents:

    1. Driver's License or State ID

    2. Two additional proofs of residence (ie. utility bills, property tax, vehicle registration)

    3. Vaccination/Immunization records that includes TDAP

    4. TB results (within the last 12 months)

    5. Transcripts and/or report cards

    6. IEP or 504 Plan (if applicable)
