Extended Leave Request

Please complete the Extended Leave Request Form available here to request an absence of more than 10 working days. Submission of the form initiates the review and approval process. It is understood that at times, employees face unexpected emergencies. To the extent possible, it is requested that the Extended Leave Request Form be submitted at least two weeks in advance of the start of an extended leave. Advance notice provides a better opportunity to gain all approvals prior to the start of the leave, ensure employees are informed of options as they relate to potentially accessing leave banks and enables Human Resources to coordinate substitute coverage when possible.
