Please include the signed (AUTHORIZATION TO RELEASE INFORMATION) along with:

1. Name you attended with at Oceana High School 
2. Date of Birth
3. Last date you attended Oceana High School.   

Please allow 24-48 hours turnaround time for the information to be faxed back.
